To comply with new GDPR legislation coming into effect in May 2018 new and exisiting clients must give "affirmative consent" for me to hold information concerning them. All existing clients will be asked to sign a privacy notice and consent statement form at their next appointment (new clients will be asked to sign this form at their first appointment).
Information that I request includes medical history and past treatments (requested to provide the best possible treatment and advice in future). All information provided by clients remains the property of the client and the client has the right to request that all or some of the information I hold is deleted. Please be aware that should you request information such as medical history be deleted you waive your right to any future insurance claim.
